During this visit, a licensed health care professional from Signify Health® will come to you and make sure you get the care you need. What to expect at a Healthy Home Visit
This checkup is an annual opportunity to meet with a licensed clinician – all at no additional cost to you.
During this visit, the clinician can:
- Check your vital signs and reflexes
- Review your medical history and medications
- Provide tips for home safety
- Check your breathing, eyes or feet
- Screen you for certain health problems
- Share details about your health plan and benefits
- Answer your health questions
- Talk about cold, flu, respiratory syncytial virus (RSV) and COVID-19 prevention
A Healthy Home Visit is a safe and friendly check-in with a trustworthy health care professional, but it doesn’t replace your relationship with your primary care provider (PCP). The Signify Health professional will share a summary of the visit directly with you and your PCP, who will use the information to help create your future care plan.
Another way to get the care you need

Aetna works with the experienced team at Signify Health® to get you safe, preventive care in-home. Once you schedule your HHV, you’ll get a confirmation notice for your appointment. You’ll also get an email, text or call reminder 24 hours before your visit. If we don’t hear from you to set up your appointment, a member of the Signify Health team will reach out to help schedule your visit.
