Say hello to your Aetna care team
This group of health professionals is here to help guide you through your health questions and concerns — with a goal of saving you time and stress along the way
Did you know you have a care team ready to help you get the right care for your personal needs? This team of dedicated health care workers can make a difference in your well-being. They can help you create a personalized care plan, schedule medical appointments, understand your medications and more. They’ll even help guide you to the benefits and local resources that best serve your health. These might include access to healthy foods, safe housing and education.
Here’s a breakdown of who might be on your care team and how each team member can help you along your health care journey.
Nurse care manager
Your care manager is a registered nurse. They can help create a full care plan that meets your health and wellness needs. And they can help put that plan into action with your primary care provider (PCP).
Social worker
A social worker helps with basic needs that affect your health such as:
- Housing
- Utilities
- Healthy groceries
They can also help answer questions about social services such as food assistance and affordable housing. And they can connect you with the right programs.
Care coordinator
A care coordinator understands all your Aetna Dual-Eligible Special Needs (D-SNP) benefits and can provide you with the best ways to use them. Here are some examples of how they can help:
- Scheduling a medical appointment
- Getting authorization for home medical equipment and medications

Aetna® Dual-Eligible Special Needs Plan (D-SNP) members can call our 24-Hour Nurse Line at
1-866-409-1221 (TTY: 711) to speak with a licensed nurse, 24 hours a day, 7 days a week.
Member advocate
Member advocates are knowledgeable about Medicaid. They can help you apply for Medicaid benefits or recertify.
Pharmacist
At least once a year, a pharmacy team member will review your medications and recommend changes (if needed) to your PCP or nurse care manager.
Health survey specialist
This team member will help you complete your yearly health survey. The survey will ask questions about you, such as your current conditions and health history. It helps your care team better understand your health needs so they can create the best possible care program for you.
Behavioral health care manager
Mental health is just as important as physical health. This care team member can help you get behavioral (mental) health support if or when you need it.
