Your D-SNP benefit checklist

Believe it or not, the new year is right around the corner. It’s not too late to use your plan benefits. We want to help you use your benefits now so you can feel your best during the holidays and start 2025 strong.

Use this checklist to keep track of the benefits you’ve used and still need to take advantage of this year.

1. Extra Benefits Card

You can use the monthly benefit amount (allowance) on your Aetna Medicare Extra Benefits Card to help pay for certain everyday expenses like: 

  • Healthy food

  • Over-the-counter (OTC) health and wellness products

Make sure you use your card allowances before the end of every month. You can use your allowance at participating retail locations, online, or by phone.

2. $0 copay on covered prescription medications

Taking your medications correctly can help you manage your chronic conditions and feel your best. And covered Part D prescription drugs (like diabetes and certain heart disease medicines) have a $0 copay at in-network pharmacies.

Is your pharmacy in-network? Or are you searching for a new pharmacy closer to home? You can easily search to find the information you’re looking for. Visit aet.na/6-oct24 and enter in your ZIP code to find a pharmacy near you. Take advantage of this benefit by remembering to pick up your prescriptions for your unique health needs. Or visit aet.na/7-oct24 to see if your prescriptions are covered. Visit the site and look for the words “Get the formulary (drug list)". 

3. SilverSneakers® fitness benefit

Have you been getting your daily exercise? Aetna members have a SilverSneakers fitness benefit that can help you keep moving. You can use it for in-person and online fitness classes (like yoga classes) and more at no added cost. Visit SilverSneakers.com/GetStarted to learn more and sign up today. Some good exercise options include:

  • Stretching
  • Dance
  • Yoga

Don’t forget to complete your annual health survey

Doing so helps your care team get you access to the benefits and resources you need. Visit AetnaMedicare.com/MyHealthSurvey to complete your health survey online. Or call your care team.

4. Covered annual exams

Staying up to date on your medical exams and checkups is important for your overall health. Aetna members have the following services available to them at no added cost. Make sure you schedule yours if you haven’t already. Can’t visit your provider in person? No problem. You can get care from the comfort of home with telehealth — for certain services with participating providers.

  • Annual wellness visit. This is a covered yearly visit with your primary care provider (PCP). It includes a personalized prevention plan and routine checks such as your height, weight and blood pressure.
     
  • Annual routine physical. This is a covered yearly exam that may include bloodwork and other important tests.
     
  • ​​​​​​​Yearly hearing appointment
     
  • Yearly eye exam
     
  • ​​​​​​​Yearly dental checkup​​​​​​​

5. Fall prevention allowance

Falling in the home happens more than you think. D-SNP members get an annual allowance to use to make your home safer. Use it to buy approved fall-prevention safety items like no-slip bathmats and grab bars. Make sure you take advantage of this allowance before the year ends to help fall-proof your home.

6. Healthy Home Visit

We work with the experienced team at Signify Health® to get you preventive care in-home. When you schedule your yearly Healthy Home Visit (HHV), a licensed Signify clinician will come to your home to help with your overall health needs. They’ll answer your health questions and share a summary of the visit with your primary care provider (PCP) to keep them in the loop. This gives you an extra opportunity to get the care you need. It’s time to renew your Medicaid!

Remember to renew your Medicaid to keep your benefits. Aetna® Dual Eligible Special Needs Plan, or D-SNP, members must renew their coverage every year to stay enrolled in their health plan.

7. Covered screenings and vaccines

Aetna members have many covered screenings and vaccinations they need for better health. Make sure you set up these important appointments if you haven’t already.

Flu vaccine. It’s never too late to get your annual flu shot. This covered vaccine is key for keeping illness away when the weather turns colder. Other vaccines like COVID-19, pneumonia and RSV are also covered by your plan. Ask your PCP if you’re due to get these shots.

Mammogram. These breast X-rays screen for breast cancer and are covered by your health plan. The American Cancer Society recommends women getting a mammogram every one to two years starting at age 45.* Doctors may recommend mammograms for men who are high-risk for breast cancer.

Colonoscopy. The American Cancer Society recommends getting a colonoscopy every five to 10 years starting at age 45.* These important screenings are covered by your D-SNP. Or take advantage of the covered in-home testing option Cologuard®.

Prostate Specific Antigen (PSA) test. PSA is a protein made by prostate cells. A PSA test can help catch prostate cancer in men. Your PCP may tell you to get tested every year, two years or more, depending on your risk. 

8. Care team services

As an Aetna member, you have services available to help guide you to better health. Your personal care team is ready to help you get the health care you need, when you need it. Call them at 1-866-409-1221 (TTY: 711) from 8:30 AM to 5:00 PM local time, Monday through Friday. You can also call this number to connect with the 24-Hour Nurse Line at any time for answers to non-emergency health questions.

And be sure to complete your annual health survey. Filling it out every year can help your care team make sure you’re taking advantage of the benefits and services you need. If you haven’t taken your survey yet, call your care team today.
